Response rates were calculated as the number of physicians who signed up to be part of this study divided by the total number of invitations sent out. Participation rates were calculated as the final number of eligible physicians who were sent a package of surveys divided by the number of physicians who signed up to participate in the study. Cooperation rates were calculated as the number of physicians who returned completed surveys divided by the total number of physicians who received surveys.
